Foster Acupuncture

Fill out any/all Required Intake Forms.

Please ensure that all required intake forms are completed before arriving at the clinic. You will receive an email with a link to our electronic intake form when you book your first appointment.
If you are unable to complete our online form, please contact Dani for a paper copy that can be printed and completed prior to your appointment.

Arrive at Your Scheduled Time.

If you arrive early please wait in your car. You may use the online self check-in tool or call 604-880-2044 to let Dani know you have arrived early. She will let you know if it is safe to come up..
Otherwise, try to arrive within 5 minutes of your appointment time and please wait in the hallway outside the doors to Suite 300. Dani will come to get you for your appointment.

Enter the Office Alone (if possible).

Only parents/guardians of minors seeking treatment or those needing assistance are permitted to have a guest with them during treatment at this time.
Please inform Dani by phone or email prior to your appointment if a guest will be present so they can also be pre-screened for COVID-19.

Perform Hand Hygiene.

All patients are asked to wash or sanitize their hands at the start and end of the appointment.

Use the Washroom Before You Arrive.

Public washrooms are available for use; however, you are encouraged to use the washroom before you leave home to reduce the areas of possible exposure.
Please note that Foster Acupuncture is located in a shared office space so although Dani can ensure her treatment room has been sanitized, she does not have control over common spaces such as the washrooms.

We encourage you to add your credit card to your online booking account to allow for automatic billing following your appointment. Click here for instructions.
We also accept Interac E-Transfers sent to dani@fosteracupuncture.com, if you prefer.
All receipts will be issued electronically at this time.

Important Considerations

It is impossible to provide acupuncture treatment while maintaining appropriate physical distance. This means that despite all of the abovementioned precautions, it is impossible to eliminate all possible risks involved with in-person acupuncture treatment. Please consider your own tolerance of risk when considering visiting the office.
