Complete Your Home Visit Application.

To ensure safety for all, mobile acupuncture appointments (home visits) can only be booked after your home has been pre-approved.
Please contact Dani at 604-880-2044 or dani@fosteracupuncture.com to request a home visit application if you have not completed one.
Approval is based on the home, not the individual patient, therefore if your home is approved, you will be able to book home visits for any/all members of your household.

If You Are a New Patient You Must Attend a Video Consult First.

First Visit Mobile Acupuncture appointments are split into 2 parts within 72hrs.
If booking online, please book Part 1 first and Part 2 within the 72hrs following Part 1. If you need assistance, please contact us and we will be happy to help you schedule these appointments.
Part 1 is a Video Consult (30 min). Having this consultation via video allows for conversation without masks and minimizes the amount of time that Dani will be in your home for your first treatment. For more details on how to join your online appointment, please click here.
Part 2 is a Home Visit with instructions below.

Complete the COVID-19 Pre-Screening.

You will receive an online survey 24hrs prior to your appointment time.

If you are unable to complete the survey online, please call Dani at 604-880-2044 and she will complete the screening with you by phone.
If your screening results indicate you may be at risk of COVID-19, please contact us to cancel your appointment. You will not be charged a cancellation fee. You will be asked to visit the BCCDC website for self assessment support or call 8-1-1 if you have not done so already.
By visiting your home, Dani will be potentially exposed to all members of your household. If you suspect any members of your household may be at risk of COVID-19 as per the questions provided in the survey, please contact us to cancel/reschedule your appointment.

Fill out any/all Required Intake Forms.

Please ensure that any/all required intake forms are completed before Dani arrives at your home. You will receive an email with a link the intake form(s) when you book your first appointment.
If you are unable to complete our online form(s), please contact us for a paper copy that can be printed and completed prior to your appointment.

Prepare Your Home.

Please ensure that the path into your home is clear and that the space we will be using for treatment is clean and cleared to allow for equipment to be set up.
Please make sure you have a heating pad that can be prepared while Dani sets up her equipment (bean bag, hot water bottle, etc). Dani will not have her TDP heat lamp with her so you will want the heating pad to stay warm and comfortable.
Ensure you have at least 2 pillows and a small blanket nearby that can be used for your comfort during treatment.
For safety reasons you will need to ensure that any/all young children and pets are restricted from the room/space for the duration of your treatment.
Any equipment that will be brought into your home is disinfected prior to arrival.

Wear a Face Mask (if possible) & Wash Your Hands.

Patients are required to wear a face mask during their appointment.
If you do not have one, Dani will give you a single use one.
If you are unable to wear a mask due to an existing medical reason, please contact Dani prior to your appointment to ensure that other arrangements can be made to increase safety for both of you during treatment.
Dani will be wearing a medical procedure mask and eye protection for the duration of your treatment. She will also change into clean/sanitized shoes at the door for use while in your home.

If you have an easily available sink close to the treatment space that you are comfortable letting Dani use, she brings her own soap and towels. If you prefer, she will use hand sanitizer instead.

We also accept Interac E-Transfers sent to dani@fosteracupuncture.com, if you prefer.
All receipts will be issued electronically at this time.

Important Considerations

It is impossible to provide acupuncture treatment while maintaining appropriate physical distance. This means that despite all of the abovementioned precautions, it is impossible to eliminate all possible risks involved with in-person acupuncture treatment. Please consider your own tolerance of risk when considering visiting the office.

Help Reduce the Spread of COVID-19

Maintain physical distancing.
If you need to cough or sneeze, do so into your elbow or a tissue. Immediately sanitize your hands after.
Avoid touching your eyes, nose and mouth.
Stay home if you are sick.
